How to request your health care records, and where to go with questions or concerns.

At Haven Psychology Group, we want clients to know how to access their records and where to go with questions or concerns.

How to Request Your Records

A records request has to include enough detail to identify you — your name, date of birth, and which records you are asking for. That is sensitive information, so please send it through one of these three routes rather than by ordinary email.

Secure message through the client portal. If you have portal access, this is the simplest route. Sign in and send a message to the administrative team.

By phone. Call (813) 438-4701 and the administrative team will take the request and confirm what is needed. This route is open to everyone, including former clients whose portal access has lapsed, and to parents or representatives requesting on someone’s behalf.

By mail. Haven Psychology Group, 28210 Paseo Drive #190, Suite 255, Wesley Chapel, FL 33543.

Please include your full name, date of birth, contact information, and a description of the records you are requesting. If you would like records sent to another provider or person, please include that as well. A signed authorization may be required; where it is, Haven will send you a Release of Information form through the client portal, by email, or by fax, whichever suits you.

Please do not send your date of birth or clinical details to admin@haven-psychology.com. Ordinary email is not a secure channel, which is why it is not one of the routes above. If you have already emailed us, call and we will pick it up from there rather than ask you to start again.

We respond to records requests in accordance with applicable state and federal law. If you would prefer your records sent to you by ordinary email despite the risk, say so and Haven will confirm that you understand it before sending them.
